Defrag operates but does not defrag, how can I fix it?

Oper. Sys. = Windows XP pro. DELL Dimension E510 Two weeks ago the defrag started a crazy act. I click on the defrag to defrag the system and the computer goes thru the usual gyrations, as if all is A OK, but finally at the end when it is finished defragging I click the analyze button. Guess what? It says, my computer needs to be defragged. I again defrag, but it does the same thing. By the way both bars have about 33% red in them. RSVP Thanks and have a good day.
