Mend Panasonic MC-785 Motor Circuit Board Wiring

How to wire my Panasonic Vacuum?

I took my vacuum apart and do not remember how to put the wiring back the way it was. I have 2 black wires to the switch, 2 wires for the circuit board (1 blk 1 wht), 2 wires from the motor (one with white stripe), 2 wires from the plug cord and 1 jumper wire. I have to put these back together with the 3 wire nuts that originally connected them. Thanks.
