agitator noise?

Kenmore HD 60 series model 110 #81864800 serial # C90809297 agitator is making a knocking sound and throwing the washer off balance during spin cycle. What could be causing this? Also noticed it seemed to have chewed a hole in a cotton shirt as if it got hung on something.
