why does the chain keep falling off the bar?

My Poulan 35cc 16" chainsaw can not keep the chain on the bar. I adjust the tension (is is an adjusting wheel, no tools required), making sure that the bottom of the chain is making solid contact with the bar. I begin to cut again and the chain comes off. Why is this happening? Do I need a new chain? How do I tell if the tensioner is bad?

sounds like the bar and chain have worn due to incorrect function of automatic chain oiler.replace both and check auto oiler for chain
