The key word is PREVENTION!

You need a "Time to crack" longer than 100 years for the best cracking gear with the worst encryption.
Any secret question needed to do a password reset must be realy secret. Someone else should find it impossible to guess it, or find it after some research. Your mother maiden name is NOT a good choice as it can be found on a descent genealogy site... But that same question with the name of a favorite pet would be OK.
For your debit card, it's possible that it got cloned.
If you notice some additional "accessory" or unusual details on an automatic teller, it's probable that there is some device added to steal the card code and your NIP.
Try to rip off the attachment (if it comes off, then IT IS fraudulent) or use another machine.
Using the informations from a cloned card, the crook could have asked for an e-mail address change, do the transactions, answer the confirmation e-mail, then, reset that e-mail to the original one.
As for the NIP, it must be impossible to guess. Your birth date or street address are horribly bad choices. Get a 10 faced die from some gaming shop and use it to roll your next NIP.
