There is not a simple answer to your question.
The 9150T manual " a green circle super-imposed on a yellow rectangle" is “displayed only when the recording is stopped because of an unexpected error”.

Some of the recorded programme should have been recorded. (A similar yellow and red icon is displayed if none of the recording is available).
My 9300T (similar to 9150T) is away at the moment, but I thought that with some versions of the Humax software it displayed a pop-up saying that the recording has failed but asking if you want to play it anyway. If you get this pop-up then opt to play it anyway.
There are a range of possible reasons that causes the yellow and green icon to be displayed
Check if you had any other timers set which could have clashed with your failed recorded programme.
The most common reason for the yellow & green icon is when the Humax thinks that there is a clash of timers.
Check the available space on your Humax’s hard disk drive.
To do this it’s MENU >> Record >> HDD Control.
Like a PC the amount of free space should be at least 20%. If it is less than this then delete recorded programmes until the free space is more than 20%.
Check that you are not tuned to more than one transmitter.
The easiest way to be tuned to only one transmitter is to follow one of the on-line forums that guide you through a manual retune.

Check which software version your 9150T is running.
To do this it’s MENU >> System >> Status. The second line of the gobbly gook gives the software version. Ideally it should read HPTTF 1.00.23 or 1.00.26. Version 1.00.26 is known to give more yellow+green icon errors than 1.00.23 but in the limited details you have supplied this does not appear to apply in this instance.
