Mend Vauxhall Combo Problem 2nd 3rd

How can I fix this problem on My 05 Vauxhall Combo?

For the past week now, every time i change from 1st to 2nd, then 3rd in my 05 Combo, the throttle seems to be sticking and continuing to rev for 5 seconds or so and is making my gear changes all jerky. There's no problem with the way it's driving but it's obviously not right.
