Mend Vauxhall Vectra Diesel Engine Power Loss Egr Valve

Why do these lights keep appearing on our vauxhall vectra ?

the egr valve and a light with a car and a spanner in the middle on the dash keeps comming on. It has been to the mechanic a number of times and the valve as been changed a number of times, also it goes into limp mode for a few seconds and loses power. This as been on going for 12 months. It is diesal engine.
