my pajero sport has asqueak everytime i press the accelerator. the belt and?

mitsubishi pajero sport 2010 is frequently back in the garage to solve the problem of an annoying persistent squeak when the accelerator is depressed. belt and tensioner have been changed but it still persists. What could be causing it?
