how can I fix my ldv pilot?

why does my engine, gearbox, van, shake violenty when I select
1st gear and let the clutch out, so violently I am unable to hang on to the gearstick, its easier if I give it higher revs, but
2nd gear grinds in high revs, 1st and 3rd sometimes engage with a clunk,problem seems worse when engine has warmed up
after a few miles, is bad on hill starts and in slow moving traffic
when trying to move on slowly, thrust bearing sounds noisy,the
ldv pilot is 1900 pergeot an ex post office with 34000 on the
clock.
