this could be 1 of manythings if this is the common rail engine there is a metering valve under the inlet manifold that plays up also the egr valve or the pressure switch on the rail itself get the van scanned and get the codes off the obd

Is it a gas or diesel ? Gas engines have resistor wire to drop the coil voltage in `Run' but by pass it in `Start' ; therefore an engine will start and fire with the key in start position then die in run if this wire has burnt out; older Toyotas used to have a stainless steel resistor which was easy to jump out as a get home move.

i have a similar problem in mine , possibly the timing chain has slipped , or the rockers may have been damaged ,get this checked , is it smokey when u try to start it ?
