how can i fix my transit 125 350 on 51 plate?

There should definitely be four plugs and it sounds like someone has had a go to do these and left one broken in, I remember a guy at a local workshop having the same problem as yours and he ended up taking the head off to drill out and tap the threads to do the job properly as access could be severely restricted on these.
