fan belt?

primestar 1.9 fan belt fell off engine still running switched off changed fan belt wont start had van in to auto elec they say mechinacal but engine was still running fot 4 mins after it fell off fan belt was whole

why did belt "fall" off-there must be a reason! is idler/tensioner intact and bearings ok, are all other items driven by fan/drive belt in line and bearings in these ok? if a portion of belt has shredded then it could have entered timing cover and got between timing belt and gears putting valve timing out!
