Engine management fault showing P400 ?

The EGR valve has been checked, a new Man sensor has been fitted but still problems.
Loss of power especially when going upi hill. EML comes on and goes off but the problem always seems to be there, all pipes have been checked and none are split.
Could it be anything to do with the VAC.
