Vauxhall Vivaro jolts forward ( losing power) when driving?

My husbands van jolts forward when he is driving everytime after he changes gear?? Its frustrating as the van ticks over lovely no loud funny noises?? We thought it might have water in the fuel tank so weve changed the filters but still the same??? Any ideas would be great
