Mend Scantronic 9300 Door Replacement Switch

I have a Scantronic 9300 alarm?

During a wooden door replacement a circular 5 x screw reed door switch got crushed. On new pvc door need to replace with surface mounted reed switch- but how to wire it in? yell,blk,red,blu wires- 5 x screws-any help please?
