Mend Fiat Ducato 2.8 tdi 2000 Model Problem

Fiat Ducato 2.8 tdi 2000 model -gearbox problem?

I can not get 5th gear on my fiat ducato motorhome,
prior to this the gear lever had a high pitched rattling sound, all the other gears work ok.
Is the 5th gear connected by an external linkage or will the engine and gearbox have to come out and the gearbox be stripped down?

the fifth gear is an overdrive add on .it can be changed without removing the gearbox.find a good mechanic.
