hot water problem?

i buy revenheat boiler just 5month befor model he 120(t) low nox silver
when boiler on after 5 min he show on display 04 even i change dhw/ch sensor aswell when i turn off main switch & then turn on again he is working propley for just 5 min after that same story plz anyone help me
