Mend Modena Remove Replace Ceramic

How do I remove the headgear from a Modena bath shower mixer?

I can replace the ceramic disc cartridge in my dripping Modena tap if I can only get at it! There doesn't appear to be any screw or other fixing; should I just lever it off with a knife, or what?
