It happened to ours.....You could be adding too much water into a system that has trapped air in it? due to a LEAK IN YOUR SYSTEM for instance: starting with a pressure reading of under 1bar with trapped air in the system (radiators) you top it up to 1.5bar the water heats up it expands it can't fill the air gap so the pressure increases more than usual it could even take your pressure over 3bar therfore opening the pressure realief valve and letting water out (pipe to the outside wall). water cools pressure drops, not allways as low as to the pressure you started with prior to topping it up, the PRV closes..... this is where your topping it up once a week comes from. note: if the PRV is not closing properly this could be your leak in the first instance. if there is no other apparant leak.
