whats wrong with my clutch?

i tried to put my 1996 escort van into reverse and it cut out and the pedal hit the floor!!! is this just tyhe clutch cable?

sometimes cable becomes detached from clutch arm -if so,refit it and secure it with a cable tie,[there is a small return spring on pedal that is supposed to keep slight pressure on it to stop this happening].

Its either the cable itself or more likely the self adjuster mechanism at the top of the clutch pedal itself has failed.
