My car's engine is noisy??

I have a Toyota Estima Lucida 92 model. The car is sounding like a tractor running and sometimes it'll only start second time round. We have been told it could be the tappets? Or is it the motor? I have five children and desperately need this fixed but dont wanna be ripped off. HELP PLEASE!!
