Mend Fiat Punto 1.2SX 75 1995 Problem Wrong

Fiat Punto 1.2SX 75 1995 steering/suspension problem?

I have a 1995 Fiat Punto 1.2SX 75 and i think there is something wrong with the steering/suspension because everytime i hit a bump or pot hole when on a slight bend or sometimes even on a straight the back end seems to slide out to the side! Ive checked the tyre pressure and its fine so nothing to do with under-inflated tyres. Whats going on with my car?
