OK I've done it!
The inside trim needs to be prised from the front to release a plastic push in prong. Although it's push fit, its VERY tight.
I actually broke the prong doing it. Luckily it doesn't really matter if you do, because once everything is screwed tight again you can't tell the difference.
Underneath the trim are three 7mm bolts (not screws) holding the mirror on. Remove those and pull the mirror off from the outside.
If you're buying a second hand replacement make sure that it comes complete with the exterior trim.
Just bolt on the replacement and screw everything back together making sure to locate the top of the door panel in the clips.
