puk is the sim you have blocked not the phone,,contact ur service provider(eg..o2..orange..etc)most u ring up or visit thier website,they just ask your tel number,then give you the 8digit code.
orange i know ring 450 select puk unlocking code when asked in the options,or visit thier site.
