do you have glass fuse with fittings for the carl lewis ?

I need a glass covered fuse for my treadmill. This fuse has blown and the treadmill cannot work without it. I also need the on/off switch was sits beside it. The model no. is MOT566 It is a motorised treadmill, the input voltage is AC23OV-50hz 1.25w, the production date is 2007/10 and the serial number is MFC000179145. I hope this information can help you to locate the parts I need.
