How can I fix my Yamada DVR-9000H DVD/HDD player/recorder?

My Yamada DVR-9000H keeps switching between recording and TV programme when playing back from the hard disk.

It also keeps changing the picture format from 4:3 to 16:9 also during playback from the HDD.

This does not happen whilst playing a DVD.

Has anyone else had the same problem and is there a solution to it?
