Mend American Standard Piece Toilet Flush

how can i fix my American Standard one piece toilet?

After a flush and the tank re-fills, the tank will re-fill on it's own. There is some down time where there is no water running, but after several minutes or an hour later, it will re-fill on it's own. What could be the problem and how do I fix it?
