Mend Hotpoint S Wrong Ice Maker

What's wrong with my ice maker?

My Hotpoint refridgerator is making ice crystals in the shute where the ice comes out and blocking the ice. It is also making ice on the bottom of the ice maker which I have to keep breaking off so the door will close. It also is making only crushed ice. (When it comes out of the dispenser). What's going on?

I have the side by side model and mine was making ice crystals around the schute too. A couple days later the fridge temp went up to high 60's. I took the panel off the back of the freezer where the coils are and everything was covered in ice there too, maybe a leak. Check it out.
