Mend Top Mixer Screw

how do I take the top off a mixer tap after I have taken the screw out?

I have taken the screw out off the tap but the top(swivel control lever ) still won't come off. I have tried w.d.40 and spraying vinigar into the hole to disolve any calc but it won't budge.
