Mend Valve Turn Replace

tap valve?

my taps are about 30yr old and when I turn it on it unscrews from the holder? When I replace it and screw it very tight as soon as I turn the tap again it frees itself again?

May be the Gland Nut / Gland Packing locked solid on the shaft/handle asembly.
