flushing problem can you help me?

I have a one year old toilet went to the higher type about a year ago never had any problems. I do have hard water not the kind that turns the porcelain red, but the lime scale is there looks almost invisible. Late last night we started having problems flushing it. When we go to flush it the water in bowl starts to swirl like it is suppose to then it rises and the swirl slows down then it goes down with out the force it had you can hear the water emptying the bowl then it fills up slowly sometimes the matter and paper will not go down and you have to flush again. I am afraid it will over flow. please help me. I live in the country and three years ago we were connected to a sewer line we have a grinder pump but if it acts up there is a light that turns on to tell us it is having problems so it isn't on and I am a summing it is my toilet. please help me I can't afford this problem right now.
