Ford Transit Connect FT39 CHANGING REAR BULBS?

I AM HAVING A NIGHTMARE.PLEASE CAN SOMEONE ADVISE FROM TOP TO BOTTOM WHAT BULBS GO IN THE SOCKETS (4 IN ALL) IF THEY HAVE 1 OR 2 ELEMENTS ETC,AS AT THE MOMENT BY SIDELIGHTS ARNT WORKING AND WHEN I PUT MY FOOT ON THE BREAK MY DASHBOARD LIGHTS UP

next one down is side light + brakelight 2 pin bulb 5 watt and 21 watt
next one down is reverse light single pin 21 watt bulb
bottom is fog light single pin 21 watt bulb

you have put the wrong bulb in the car eg wrong brake light bulb?
