I opened the wall adapter and found the fuse had blown.
I found some new replacement fuses at an electrical supply, $2.00 for a package of 5.
There was no fuse holder, the fuse was soldered directly in-line. I cut the old fuse out and soldered the new fuse directly onto the in-line wire. It as a little tricky but with very small rosin core solder and a fine tip solder gun it worked out.
Vibrator/heat work again in the recliner and I have some spare fuses.

I need one as well, for a chair that has only been used twice even though it is two years old. I called Lazy-Boy and they tell me that the part is free but the service call is $80. No offer to mail me a new one. I think LazyBoy needs to change their policy since it looks like this is a design flaw. Too many folks having the same problem.

I have the same thing and I cannot find the replacement anywhere, so I took it apart and found it had a blown fuse inside on one leg of the circuit. The fuse is a 5amp 250vac slow blow fuse and it's about 5x20mm in size, these fuses can be found at RadioShack and many other places but you need to know how to get to them and then what to do when you get there. If you can handle that then you will probably find the same problem with your adaptor.

one of this adaptors uses is for a La-Z-Boy heat and massage chair. I am also in search of one. I stopped by the local La-Z-Boy store yesterday and was given a number for their service tech. I am waiting for a call back, and if I receive a response, I'll post it here.
