Stem on hot water tap turns off but keeps turning till it comes on again , stem has been replaced and new packing, packing nut is turned as tight as it can get (taps are from 1954). Could it be the threads inside the wall????

The only way to fix ceramic taps is to remove the whole tap insert and replace it with another one...take the old one with you though as they have to be absolutely identical in size. B&Q have a variety of these. Good luck!
