You will need the GC (Gas Council) number for the boiler that's on a plate inside the casing. This will then allow you to get the correct part.
On this fan the fins on the plastic impeller cooling fan break off and then the bearing overheats leading to failure. Same fan is fitted to some of the Glow-worm boilers (Compleheat) too.
