how can i fix my ford galaxy 1.9 tdi ghia 2002. heater?

hi everyone, My galaxy 1.9 tdi, heater works sometimes. After driving a little bit, it will start to go cold. so then i have to rev the car to 4000 rpm. for it come back for about 5 min. then i have to repeat the process for it to work again. can anyone help me as to what the problem might be. I did change my thermosate recently because the water bootle would go boiling hot. so changed that , it seems to have done the job , but still the problem with the heater, and also i have to top up the water bootle every 100 miles or so, (warning light come up, with check coolent.)

possible that the water pump impellor is just spinning free and not attatched to water pump shaft , not worked on these before but id take the top off your water bottle (when engine is cold ) look inside and see if the water is "flowing"/swirling .if ya cant see take the little return pipe off to the bottle and check the flow with it off should flow nicely out at tick over (not little trickle ),get some one to raise revs 2-3000rpm and see if flow picks up shuld flow fast (point pipe in to a container or in to top of the water bottle ).if not water pump need replacing ,hope this helps .
